Getting organized and knowing the proper way to address your retirement plan can help you get quicker, more effective results. Wellington has created forms that can help you organize and manage your retirement plan. All our forms are available in one format: portable document format (PDF).

To download a form: Click the PDF button next to the file name. Depending on your browser, the form will either open immediately or instruct you to save the file to your hard drive. If you open the form directly, you can save them to your hard drive by pulling down your File menu and selecting Save As. You can also print the forms by pulling down your File menu and selecting Print.

The files will vary in size and will download in most cases in less that one minute (with 28.8 bps modem). You'll need the free Adobe Acrobat Reader to view and print the PDF forms. If you don't already have this software, you can download it at www.adobe.com/products/acrobat/readstep2.html .

  Enrollment Forms
 
ENROLLMENT FORM - This form is used when you initially enroll into your retirement plan (FORM 1) . Plan enrollment requires a completed FORM 1, FORM 3, FORM LPOA, and if applicable, FORM 2. See also the retirement planning worksheet and the asset allocation questionaire.
CHANGE OF INVESTMENT ELECTION FORM - This form is used primarily after you have enrolled in your retirement plan and you want to change salary deferral amounts or reallocate your portfolio (FORM 2).
BENEFICIARY ELECTION FORM - This form is used to select or change a beneficiary designation (FORM 3).
SCHWAB LPOA FORM - This form is used to establish your account at Schwab. This form is mandatory for plan enrollment (FORM LPOA).
  Transfer Forms
 
TRANSFER INSTRUCTIONS - This file will instruct you on the proper procedures to transfer over an existing 401k from a previous employer or a Rollover IRA into your new 401k.
  Distribution Forms
 
TERMINATION DISTRIBUTION FORM - This form is used when you terminate employment and wish to rollover your account or take a distribution from your 401k. By filling out the Schwab IRA application from below, we can establish an IRA rollover for you. You may also keep your 401(k) in place at Schwab with the option of transferring it out at a later date by doing nothing. Contact Wellington for instructions.
LOAN REQUEST FORM - Application paperwork to obtain a loan from your 401(k) plan. This option is only available if you plan allows loans.
HARDSHIP REQUEST FORM - Application to obtain a hardship distribution from your 401(k) plan. This option is only available if you plan allows this type of distribution.
  Rollover
 
ROLLOVER FORM AND INSTRUCTIONS - This Form will provide you with instructions on how to rollover 401(k) assets from a previous employer or a conduit IRA to your new 401(k).
SCHWAB IRA APPLICATION - This Form will establish an IRA to rollover your 401(k) assets when you terminate employment from your current employer. Forward your completed form to Wellington at 14325 Willard Rd. #104, Chantilly, VA 20151 .
SCHWAB IRA DISTRIBUTION FORM - This Form will allow you to request a distribution from your IRA. Simply download, print, sign and fax back to us. We'll take care of it from there.
